	Check if no Apt. Number; Private House:		Check if Apt. Number is blank
	City or Town:		City or Town required City can only contain letters or spaces
	State:	CT NJ NY	State required
	Zip Code:		Zip Code required Zip Code must be 5 digits
	Phone Number: (ex: 123-456-7890 - preferably a phone number that is able to receive text messages)		Phone Number required Phone number format is 212-555-1234
